Independent Contractor Driver Agreement - Local Meal Delivery. I understand that I am applying as a 1099 contractor responsible for my own taxes, insurance, and other delivery driver requirements. I understand I am not employed by Local Meal Delivery.

Auto Insurance *

I certify that I have and will maintain an auto insurance policy under my name for the full extent of the time I am a delivery driver partnered with Local Meal Delivery.

Call and or text every customer *

I certify that I understand that I must call and or text every customer while at the restaurant to inform the customer of my ETA, to make sure they were able to order everything they wanted and establish contact prior to dropping off food at customer location. I understand that if I do not contact customer I risk forfeiting any delivery fee or tip refunded to customer based on any issue regarding lack of driver communication. You may be asked for screenshots to prove such communication. Local Meal Delivery sets itself apart from other delivery companies by providing excellent communication during the delivery process.

Contract *

Driver accepts the following requirements as the minimum acceptable conditions of the agreement: Driver does possess a valid driver's license. Driver provides a fully insured vehicle with valid registration and inspection. Driver accepts full accident liability for personal injury and vehicular damage. Driver accepts full responsibility for any traffic violations. Driver will attempt to deliver all items in as close condition as possible as they leave the restaurant or hub. ABSOLUTELY NO SMOKING IN VEHICLE. Driver accepts full responsibility for the care and condition of any Local Meal Delivery owned equipment supplied to driver for use while performing contracted deliveries. Local Meal Delivery and the Drivers accept the following conditions of compensation: Weekly and after completion of all deliveries, Local Meal Delivery will pay the Driver for each and every delivery completed and with no customer complaints to the correct address at the correct time. Photo of each delivery is a must. For alcohol delivery, ID check and signature is required. Each address is considered one delivery regardless of the amount of the items. The compensation minimum for on-demand varies from State to State. They are $20 to $200 for each delivery and 100% of all tips. For deliveries like catering. The pay will depend on the accepted price by the providers and Local Meal Delivery. It is hereby acknowledged that Local Meal Delivery is not liable for personal injury or vehicular damage incurred while the Driver is performing duties as a contract delivery person. By submitting this application form, you are agreeing to the terms and conditions in this contract. A background check will be conducted, and also agree to receive emails and text SMS updates from our marketing team.

Term and Termination *

I understand and agree that this Agreement is effective as of the date it is signed and shall remain in effect until terminated by either party. Either party may terminate this Agreement at any time, with or without cause, by providing written notice to the other party.
